Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:30 AM - 4:30 PM with weekend rotations (may include Saturday and Sunday); Day shift
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Conduct comprehensive patient assessments and identify case management needs
- Develop and implement individualized case management plans in collaboration with patients, families, and the care team
- Perform utilization review to ensure appropriate, high-quality care and resource use
- Coordinate and lead discharge planning to ensure safe transitions across settings
- Monitor patient progress, evaluate outcomes, and adjust plans to meet goals
- Document interventions, assessments, and outcomes accurately in the EMR and meet regulatory timelines
- Collaborate with multidisciplinary teams and community partners to integrate services including behavioral health and long-term supports
- Maintain an active caseload and provide regular outreach and follow-up
Qualifications
- Current Montana RN licensure (required)
- American Heart Association BLS (required)
- Minimum 2 years of nursing experience (required)
- Utilization review experience (required)
- Bachelor of Science in Nursing (BSN) preferred
- 2 years of acute care hospital case management experience preferred
- Strong verbal and written communication skills and high emotional intelligence
- Willingness to work weekends and participate in on-call rotations as required
Education
- Registered Nurse (RN) licensure - required
- Bachelor of Science in Nursing (BSN) - preferred
Knowledge & Skills
- Patient assessment and care plan development
- Utilization review and case management best practices
- Discharge planning and care transitions
- Proficiency with EMR systems and accurate documentation
- Comfortable with MS Office and common computer applications
- Ability to collaborate with interdisciplinary teams
- Experience integrating behavioral health and long-term services
- Strong organizational skills and ability to prioritize in a busy environment
