Roles & Responsibilities

• Provide compassionate psychosocial assessments and individualized care planning for hospice patients
• Deliver patient and family counseling, including bereavement support and crisis intervention
• Conduct home visits to assess needs in the patient’s residence, assisted living or skilled nursing facility
• Coordinate care with nurses, physicians, hospice aides and community resources to ensure holistic support
• Advocate for patients and families to secure appropriate services, benefits and supports
• Document social work assessments, care plans and outcomes in a timely, professional manner
• Offer spiritual and emotional support while respecting diverse beliefs and cultural backgrounds
• Participate in interdisciplinary team meetings to enhance patient-centered hospice care

Qualifications

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) required
• BLS (Healthcare Provider) certification required
• Valid driver’s license and current auto liability/property damage insurance required
• Oregon LCSW preferred
• Experience in hospital or medical social work preferred
• Comfort with making home visits and working across a variety of care settings

Education

• Master’s degree in Social Work (MSW) or equivalent, consistent with Oregon LMSW licensure

Knowledge & Skills

• Strong knowledge of hospice care principles and end-of-life psychosocial needs
• Skilled in bereavement counseling, crisis intervention and family support
• Excellent case management and care coordination abilities
• Clear, compassionate communication and active listening skills
• Ability to navigate community resources and make effective referrals
• Proficient documentation and time management in clinical settings
• Cultural sensitivity and ability to work with diverse populations
