Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living (ADLs)
• Monitor residents' condition and report changes promptly to nursing staff
• Assist with safe transfers, mobility support, and positioning
• Maintain accurate and timely documentation of care and observations
• Support infection control and safety practices to protect residents and team
• Collaborate with nurses and rehabilitation staff to deliver person-centered care
• Build trusting relationships with residents and families through respectful communication

Qualifications

• Active Licensed Nursing Assistant (LNA) certification — VT and/or NH licensure noted in posting
• Reliable transportation required (LOCAL ONLY position)
• Willingness to work evening shift (3:00 PM - 11:00 PM) and every other weekend
• Current basic certifications as required by facility (e.g., CPR) — bring documentation
• Strong communication skills and a team-first attitude

Education

• High school diploma or GED preferred
• Licensed Nursing Assistant (LNA) certification required

Knowledge & Skills

• Familiarity with ADLs, resident lifts/transfers, and safe patient handling
• Basic clinical observation skills (vitals, skin integrity, behavior changes)
• Good documentation habits and attention to detail
• Ability to follow care plans and take direction from nursing staff
• Compassionate interpersonal skills and the ability to engage residents positively
