Roles & Responsibilities
- Assess patients' speech, language, cognitive-communication, and swallowing function using evidence-based methods
- Develop and implement individualized treatment plans to improve communication and swallowing safety
- Provide skilled therapy services in a skilled nursing facility setting with compassion and clinical excellence
- Document evaluations, plans, and progress notes accurately and timely to meet facility and regulatory standards
- Collaborate with the interdisciplinary team to coordinate care and support patient goals
- Educate patients, families, and facility staff on communication and swallowing strategies to maximize outcomes
- Meet productivity expectations and adhere to Continuum Rehabilitation Group policies and procedures
- Maintain professional appearance (black scrubs as required) and be present on-site to receive guaranteed hours
Qualifications
- 1–3 years of SLP experience (as stated: Years Experience: 1-3)
- Experience working in a skilled nursing facility (SNF) setting preferred
- Ability and willingness to follow CRG policies & procedures and meet a 90% productivity requirement
- Comfortable working on-site and following facility dress code (black scrubs)
- Strong interpersonal and documentation skills to collaborate with the therapy team and facility staff
Knowledge & Skills
- Skilled in clinical assessment and treatment planning for speech, language, cognition, and swallowing
- Proficient documentation and charting to meet regulatory and facility standards
- Excellent communication and teamwork skills for interdisciplinary collaboration
- Ability to manage caseloads efficiently while maintaining high-quality care
- Familiarity with SNF workflows and resident-centered care practices
