Roles & Responsibilities

• Complete accurate, timely OASIS documentation in accordance with agency, state, and federal requirements
• Assess patients and caregivers using the nursing process to develop and communicate realistic, evidence-based plans of care
• Serve as case manager for an assigned caseload and manage visit times per agency productivity standards
• Implement nursing interventions and procedures (including IV therapy and wound VAC care) to ensure safe, high-quality care
• Maintain infection control standards across the plan of care
• Document care thoroughly in McKesson — locking down visits, follow-up notes, and visit completion
• Provide patient and caregiver education so families can make informed decisions about treatments and discharge plans
• Coordinate care with interdisciplinary team members, vendors, and providers to support the plan of care
• Participate in peer review audits and proactively schedule case conferences to improve patient outcomes
• Respond to on-call needs, including hospice coverage when scheduled

Qualifications

• Current Registered Nurse (RN) licensure in accordance with the Vermont State Board of Nursing
• Demonstrated skill with OASIS documentation and regulatory-compliant charting
• Clinical experience with IV therapy, wound VAC, and complex wound care
• Familiarity with McKesson workflows and electronic visit documentation
• Strong case-management and time-management abilities to handle assigned caseloads
• Comfort participating in on-call rotations, including hospice coverage

Knowledge & Skills

• Thorough knowledge of the nursing process and evidence-based home health practices
• Proven infection control practices and wound care techniques
• Excellent documentation skills and attention to regulatory detail
• Ability to educate patients and caregivers clearly and compassionately
• Collaborative approach to working with LPNs, therapists, social workers, and physicians
• Ability to coordinate discharges, transfers, and specialty referrals
