Roles & Responsibilities

• Assess patients thoroughly and develop individualized outpatient treatment plans
• Deliver hands-on and evidence-based physical therapy interventions to promote function and independence
• Provide timely, accurate documentation and communicate patient progress to the care team
• Step in to cover inpatient overflow (observation, acute & SNF) when needed to ensure continuity of care
• Educate patients and families on home programs, safety, and self-management strategies
• Collaborate with interdisciplinary teams to coordinate care and optimize outcomes
• Maintain up-to-date competencies and participate in departmental quality initiatives

Qualifications

• Active New Hampshire Physical Therapist license (Compact allowed)
• Minimum 2 years recent Physical Therapy experience — outpatient experience required
• Inpatient experience highly preferred; NO NEW GRADS
• Current American Heart Association BLS certification (document required)
• One supervisory signed reference from the past year of employment (required)
• All licenses/certificates must be current and available at time of submission
• Permanent address must be included in the submission file
• Ability to provide license verification and required documentation upon request

Education

• Graduation from an accredited Physical Therapy program (entry-level PT degree required to hold licensure)

Knowledge & Skills

• Strong outpatient evaluation and treatment skills, including therapeutic exercise and manual techniques
• Comfort managing a varied caseload and adapting to occasional inpatient/overflow responsibilities
• Proficient documentation skills and familiarity with electronic medical records
• Excellent communication, time management, and patient education abilities
• Ability to work well within a multidisciplinary team and contribute to clinical problem-solving
• Clinical reasoning and goal-setting focused on functional outcomes
