Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments accurately and according to policy
• Assess and monitor patient condition, reporting changes to the care team promptly
• Document care thoroughly in the medical record to ensure continuity and safety
• Collaborate with RNs, physicians, and multidisciplinary team members to deliver excellent care
• Support residents with activities of daily living while promoting dignity and independence
• Participate in shift handoffs and care planning to maintain high-quality outcomes

Qualifications

• Licensed Practical Nurse (LPN) — current, active license required
• Clinical experience working in a long-term care or similar setting is a strong plus
• Ability to follow care plans, safely administer medications, and document accurately
• Good communication skills and a team-oriented approach to resident care
• Proof of immunizations and required facility screenings as requested

Education

• Graduate of an accredited LPN program (practical nursing diploma or equivalent)

Knowledge & Skills

• Solid knowledge of medication administration and nursing fundamentals
• Comfortable performing vital signs, assessments, and routine procedures
• Strong documentation skills and familiarity with electronic or paper charting
• Ability to prioritize tasks and manage time during a full shift
• Compassionate bedside manner and patient-focused communication
