Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents at Larkin Chase Center
• Administer medications and treatments accurately and safely, following facility protocols
• Perform timely assessments and monitor vital signs to catch changes early
• Document care thoroughly in the electronic health record to support continuity
• Collaborate with nurses, therapists, and support staff to create and update care plans
• Educate residents and families on therapies, medications, and discharge plans in a clear, respectful way
• Support a safe environment by following infection control and safety procedures

Qualifications

• Current Licensed Practical Nurse or Registered Nurse license (as applicable) — active and in good standing
• Valid BLS/CPR certification (or willingness to obtain prior to start)
• Clinical experience in long-term care, skilled nursing, or similar settings is preferred
• Strong communication and teamwork skills — you’ll be coordinating with a lot of caring people
• Ability to work the posted rotating schedule, including weekends

Education

• LPN: Completion of an accredited Practical Nursing program or diploma
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment and recognition of clinical change
• Familiarity with electronic health record documentation
• Infection prevention and control procedures
• Time management and prioritization in a fast-paced care setting
• Respectful, compassionate patient and family communication
