Candidate Pay Rate: $55 - $65 per hour

Roles & Responsibilities
- Provide physical therapy treatments under the supervision of a licensed Physical Therapist to help residents regain function
- Assist residents with therapeutic exercises and mobility training to improve independence
- Implement individualized treatment plans tailored to each resident's goals
- Monitor and accurately document patient progress to support continuity of care
- Collaborate with interdisciplinary healthcare teams to coordinate excellent patient-centered care
- Ensure compliance with facility and state regulations to maintain a safe, high-quality environment
Qualifications
- Active California PTA License (required)
- Current BLS Certification
- SNF experience preferred
- Strong clinical judgment and excellent communication skills

- Physical therapy treatments and modalities
- Therapeutic exercise design and progression
- Mobility training and fall prevention strategies
- Developing and implementing individualized treatment plans
- Accurate patient progress monitoring and documentation
- Team-based collaboration in a skilled nursing setting
