Roles & Responsibilities

• Evaluate patients to identify functional deficits and develop individualized treatment plans
• Deliver high-quality outpatient physical therapy interventions for orthopedic, sports, pediatric, and neurological needs
• Provide balance, dizziness, wheelchair seating, and neck/back/extremity care with a patient-centered approach
• Document clinical findings, progress, and discharge summaries clearly and promptly
• Collaborate with interdisciplinary teams to support transitions of care, including acute and swing bed rehab
• Educate patients and families to promote safe home programs and long-term functional gains
• Travel between multiple points of care to ensure access and continuity of therapy services

Qualifications

• Current, unencumbered Oregon Physical Therapy license required
• Healthcare Provider Level BLS required
• Comfortable traveling to multiple points of care within the service area
• Experience in outpatient physical therapy settings preferred, especially orthopedic and sports rehab
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication and teamwork abilities

• Orthopedic, sports, pediatric, and neurological rehabilitation approaches
• Balance and dizziness rehabilitation techniques
