Roles & Responsibilities
- Provide compassionate direct patient care and monitor residents’ clinical status
- Administer medications accurately and document treatments in the chart
- Perform thorough assessments and communicate changes to the care team
- Collaborate with nurses, therapists, and providers to implement individualized care plans
- Maintain infection control practices and promote a safe healing environment
- Support residents’ daily living needs with dignity and respect
Qualifications
- Active Licensed Practical Nurse credential (LPN)
- Previous LPN experience in long-term care or skilled nursing preferred
- Current BLS/CPR certification or willingness to obtain prior to start
- Reliable transportation — local assignment requires you have your own vehicle
- Ability to obtain any facility-required screenings and approvals before start
Education
- Completed accredited Practical Nursing program (LPN diploma or certificate)
Knowledge & Skills
- Strong skills in medication administration and basic clinical procedures
- Comfortable with resident assessment, documentation, and care planning
- Good communication and teamwork to coordinate with interdisciplinary staff
- Knowledge of infection control principles and safe patient handling
- Ability to learn facility-specific electronic charting systems
