Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients, ensuring dignity and comfort
• Administer medications and treatments accurately, following physician orders and facility protocols
• Monitor patient conditions and document changes in status, communicating concerns promptly to the care team
• Assist with activities of daily living (bathing, dressing, mobility) while promoting independence
• Collaborate with RNs, CNAs, therapists, and support staff to coordinate excellent patient-centered care
• Support facility workflows by accurately completing charts, medication records, and shift handoffs

Qualifications

• Current LPN license (Licensed Practical Nurse) in Pennsylvania required
• CPR/BLS certification required or must obtain upon hire
• Comfortable with medication administration, basic wound care, and documentation
• Strong communication skills and a team-oriented attitude
• Previous LPN experience in long-term care or a similar setting preferred

Education

• Graduate of an accredited Practical Nursing program (LPN/LVN)
• High school diploma or GED

Knowledge & Skills

• Clinical assessment and basic nursing procedures
• Medication administration and documentation best practices
• Patient safety, infection control, and standard precautions
• Electronic health record basics and accurate charting
• Ability to prioritize, multitask, and respond to changing patient needs
