Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients with dignity and respect
• Administer medications and treatments accurately and safely according to orders
• Monitor and record vital signs and patient status, reporting significant changes promptly
• Document patient care thoroughly in the chart and maintain clear communication with the care team
• Assist with daily living activities and mobility support while promoting independence
• Collaborate with RNs, providers, and interdisciplinary team members to support care plans
• Support a positive environment by mentoring CNA staff and contributing to unit initiatives

Qualifications

• Licensed Practical Nurse (LPN) — current, active license required
• Comfortable working in a long-term care or skilled nursing environment
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Strong communication and teamwork skills with a patient-centered approach
• Ability to pass background checks and required facility screenings

Education

• Completion of an accredited LPN/LVN program
• High school diploma or GED required

Knowledge & Skills

• Good clinical assessment skills and the ability to recognize changes in condition
• Medication administration knowledge and safe documentation practices
• Familiarity with electronic medical records and basic charting
• Skilled at infection control, wound care basics, and safe patient handling
• Excellent interpersonal skills and the ability to work well under pressure
