Roles & Responsibilities

• Provide compassionate, patient-centered nursing care and support daily living activities
• Administer medications and treatments accurately and document per policy
• Perform routine patient assessments and report changes in condition promptly
• Collaborate with RNs, CNAs and interdisciplinary team members to optimize care plans
• Maintain clear, timely and professional documentation in the patient chart
• Assist with admissions, transfers and discharges to ensure smooth transitions
• Contribute to a safe, clean environment and follow infection control standards

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable working in a fast-paced long-term care environment
• Strong communication skills and a team-first attitude
• Prior LPN experience preferred but new grads with clinical readiness are welcome to apply

Education

• Diploma or certificate from an accredited Practical/Vocational Nursing program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital signs monitoring
• Familiarity with electronic medical records and charting
• Infection control procedures and safe patient handling
• Effective communication with patients, families and care teams
• Time management and prioritization in a clinical setting
