Roles & Responsibilities

• Provide high-quality bedside care for residents at Bear Canyon Rhb Ctr, ensuring comfort and safety
• Administer medications and treatments accurately and document care in the patient record
• Assess patient status, report changes promptly, and collaborate with the care team
• Support care plans by implementing nursing interventions and monitoring outcomes
• Educate patients and families about care routines and health maintenance in a compassionate way
• Participate in shift handoffs and ensure clear, accurate documentation for continuity of care
• Uphold infection control standards and facility policies to maintain a safe environment

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico
• Completion of an accredited LPN program
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable with medication administration, basic assessments, and documentation
• Ability to pass background check and meet facility health requirements
• Local candidates preferred due to local-only assignment status

Education

• Completion of an accredited LPN program
• High school diploma or GED

Knowledge & Skills

• Strong clinical assessment and vital sign monitoring skills
• Proficient medication administration and documentation practices
• Ability to prioritize tasks and manage time during longer shifts
• Clear communication and teamwork with nurses, therapists, and support staff
• Familiarity with long-term care or rehabilitation settings is a plus
• Compassionate patient-centered approach with attention to detail
