Roles & Responsibilities

• Perform thorough admission and start-of-care assessments to set a strong plan for each patient
• Complete OASIS documentation accurately and timely to support patient outcomes and compliance
• Deliver safe medication administration and management, including IM, subcutaneous and IV infusions
• Provide skilled wound care and integumentary assessments to promote healing and prevent complications
• Start and maintain IVs and perform phlebotomy as needed for patient care
• Apply infection control precautions and educate patients and families on best practices
• Collaborate with PT, social services and interpreters to coordinate holistic, patient-centered care
• Maintain clear, professional charting in Epic and communicate changes in patient status promptly

Qualifications

• Active RN license (pending licenses accepted) with eligibility to practice in Oregon
• Minimum 1 year of skilled Home Health experience with OASIS experience required
• BCLS certification required
• Demonstrated skills in wound care, medication administration, IV therapy and phlebotomy
• Comfortable with charting in Epic (Epic experience preferred)
• Reliable transportation required (occasional patient visits may be within a wide radius)
• Able to work occasional weekends when needed

Education

• Active RN license (or pending license accepted)
• BCLS certification

Knowledge & Skills

• Strong knowledge of Home Health documentation standards, including OASIS
• Proficiency with Epic electronic charting
• Clinical skills: patient assessment, medication administration, IV therapy and phlebotomy
• Wound care assessment and treatment skills
• Infection control best practices and patient education
• Ability to manage multiple patient visits per day while maintaining quality care
• Comfort working independently in community and home settings
