Roles & Responsibilities
- Administer medications and treatments safely and accurately, following facility protocols
- Monitor patient condition and report changes promptly to the care team
- Provide compassionate, person-centered nursing care in a skilled nursing/rehab setting
- Document assessments, interventions and care plans clearly and timely in the chart
- Support patient mobility and rehabilitation goals in collaboration with therapists
- Assist with admissions, discharges and smooth transitions of care
- Educate patients and families on care plans, medications and safety measures
- Participate in interdisciplinary team meetings to improve patient outcomes
Qualifications
- Current LPN or RN license in good standing (Massachusetts licensure preferred)
- Valid BLS/CPR certification
- Experience in long-term care or rehab nursing preferred
- Current immunizations and ability to pass background and health screenings
- Strong clinical judgment, compassionate patient care, and teamwork mindset
Education
- Completion of an accredited LPN program or an accredited ADN/BSN program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and pressure injury prevention
- Familiarity with electronic medical records and charting
- Ability to prioritize, multitask and communicate clearly with patients and staff
- Infection control principles and safe patient handling
