Roles & Responsibilities

• Provide compassionate, high-quality physical therapy assistance to adults and geriatric patients in their homes
• Deliver targeted orthopedic and geriatric interventions using evidence-based therapeutic techniques
• Document visits accurately and promptly in EPIC (remote), Rover (mobile), and OASIS to support continuity of care
• Manage an efficient caseload — you may visit up to 6 patients per day while working toward the 30 productivity points standard
• Coordinate care with nurses, therapists, and the interdisciplinary team, using interpretation services when needed
• Plan safe travel routes and use your own transportation responsibly to ensure timely visits
• Adhere to facility policies including infection control and required vaccinations

Qualifications

• Minimum 1 year of skilled home health experience required
• Current BCLS/BLS certification required
• Active PTA license preferred; pending licenses accepted (confirm requirements with manager)
• Comfortable and experienced with EPIC, OASIS documentation, and Rover mobile charting
• Reliable personal transportation required
• Experience or interest in geriatric and orthopedic patient populations
• Willingness to meet facility vaccine requirements (primary COVID series plus one booster)

Knowledge & Skills

• Proficient documentation skills (EPIC, OASIS, Rover)
• Strong clinical skills in home health therapeutic interventions
• Excellent time management and route-planning abilities
• Clear communication and teamwork with interdisciplinary staff
• Comfort using a laptop and mobile app for charting and communication
• Ability to adapt to varied home environments and patient needs
