Roles & Responsibilities

• Conduct thorough psychosocial assessments to identify patient needs and risks
• Provide crisis intervention for high-acuity patients with empathy and clear clinical judgment
• Coordinate multidisciplinary care to ensure seamless, patient-centered treatment plans
• Navigate housing and public benefits systems to connect patients with stable resources
• Deliver substance use and mental health counseling tailored to each patient’s situation
• Document clinical encounters clearly and efficiently to support continuity of care
• Communicate effectively with patients, families, and clinical teams to advocate for high-risk patients
• Support case follow-up and transitions of care to reduce readmissions and improve outcomes

Qualifications

• Master’s degree in Social Work (required)
• Eligibility for California clinical social work licensure (required)
• Ability to work independently in high-volume, high-acuity ambulatory settings
• Strong documentation and communication skills
• Preferred: LCSW or LMFT credential
• Preferred: HIV Test Counselor credential
• Preferred: Fluency in Spanish
• Preferred: 2-4 years’ experience providing social work services to HIV-positive populations
• Facility requirement: COVID-19 vaccination and booster (medical/religious exemptions only)

Education

• Master’s degree in Social Work (MSW) — required

Knowledge & Skills

• Skilled in psychosocial assessment and crisis intervention
• Experienced with housing and benefits navigation for vulnerable populations
• Knowledgeable in substance use and mental health counseling approaches
• Proficient in multidisciplinary care coordination and clinical documentation
• Clear and compassionate communicator with patients and care teams
• Culturally responsive care, including Spanish fluency as a plus
• Comfort working in fast-paced, high-volume clinical environments
