Roles & Responsibilities

• Evaluate residents' speech, language, cognitive-communication, and swallowing function using evidence-based assessments
• Develop and implement individualized, measurable treatment plans that improve quality of life
• Collaborate with nursing, rehab team members, and families to coordinate care and set meaningful goals
• Document evaluations and progress notes promptly and accurately to support continuity of care
• Provide skilled therapeutic interventions and caregiver education to maximize resident independence
• Participate in interdisciplinary meetings and contribute to resident care planning

Qualifications

• Licensed or license-eligible Speech-Language Pathologist (SLP) — state licensure requirements apply
• ASHA CCC-SLP recognized (CCC indicated in job title/requirements)
• Open to clinicians at all experience levels, including new graduates
• Experience or comfort working in a skilled nursing facility (SNF) setting is a plus
• Strong clinical reasoning, documentation skills, and ability to collaborate with an interdisciplinary team

Knowledge & Skills

• Skilled in instrumental and bedside dysphagia management and safe swallowing strategies
• Proficient in speech, language, cognitive-communication assessment and therapy techniques
• Comfortable with resident-centered plan development and outcome-driven documentation
• Excellent interpersonal skills for working with residents, families, and clinical teams
• Time management and organizational skills to balance caseload and documentation
