Roles & Responsibilities

• Evaluate residents' physical function and develop individualized treatment plans to restore mobility and independence
• Deliver evidence-based therapeutic interventions in a skilled nursing setting to improve strength, balance, and gait
• Collaborate with nurses, occupational therapists, speech therapists, and family members to coordinate goal-driven care
• Document resident progress, plan updates, and discharge recommendations clearly and timely
• Educate residents and caregivers on safe mobility techniques, fall prevention, and home exercise programs
• Participate in care conferences and contribute to interdisciplinary planning to optimize outcomes

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — or eligible for MN licensure
• Comfortable working in a Skilled Nursing Facility (SNF) environment
• Open to clinicians at any experience level, including new graduates
• Strong communication and documentation skills to meet regulatory and facility standards

Education

• DPT or accredited Physical Therapy degree preferred

Knowledge & Skills

• Skilled in PT evaluation and treatment planning for older adults
• Knowledge of SNF care processes, restorative nursing, and discharge planning
• Ability to document thoroughly in electronic clinical records
• Effective interdisciplinary communication and patient/family education skills
• Clinical reasoning to progress residents safely and efficiently
