Roles & Responsibilities
- Evaluate residents' physical function and develop individualized treatment plans to restore mobility and independence
- Deliver evidence-based therapeutic interventions in a skilled nursing setting to improve strength, balance, and gait
- Collaborate with nurses, occupational therapists, speech therapists, and family members to coordinate goal-driven care
- Document resident progress, plan updates, and discharge recommendations clearly and timely
- Educate residents and caregivers on safe mobility techniques, fall prevention, and home exercise programs
- Participate in care conferences and contribute to interdisciplinary planning to optimize outcomes
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — or eligible for MN licensure
- Comfortable working in a Skilled Nursing Facility (SNF) environment
- Open to clinicians at any experience level, including new graduates
- Strong communication and documentation skills to meet regulatory and facility standards
Education
- DPT or accredited Physical Therapy degree preferred
Knowledge & Skills
- Skilled in PT evaluation and treatment planning for older adults
- Knowledge of SNF care processes, restorative nursing, and discharge planning
- Ability to document thoroughly in electronic clinical records
- Effective interdisciplinary communication and patient/family education skills
- Clinical reasoning to progress residents safely and efficiently
