Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (day shift; 8-hour days, start time flexible between 6:00 AM and 10:00 AM; 35 GWW)
Hours per Week: 35
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Assess and evaluate patients’ physical functioning and wound care needs using standardized assessments
- Develop individualized, evidence-based plans of care that meet regulatory and clinical practice standards
- Provide hands-on rehabilitation interventions: manual techniques, therapeutic exercises, modalities, ambulation and DME recommendations
- Track patient progress, adjust treatments, and document outcomes accurately in the electronic record
- Collaborate with the interdisciplinary team to coordinate care, referrals, and discharge planning
- Provide patient, family, and caregiver education and counseling to support recovery and safety
- Supervise and mentor PTAs, rehab aides, and students while maintaining professional standards
- Participate in facility and company-required meetings, trainings, and quality initiatives
Qualifications
- Missouri state physical therapist license required (Compact allowed)
- Minimum 1 year of PT experience required
- Level II clinical experience required — New grads okay if Level II (not just Level I observation)
- LTC or inpatient/outpatient rehab experience required
- Strong clinical documentation skills and accurate recording of treatment minutes
- Ability to obtain necessary DME/supplies and coordinate procurement per regulations
- Adherence to APTA and state practice acts, professional code of conduct, and ethics
- Comfort supervising PTAs, aides, and students and providing clinical mentorship
Knowledge & Skills
- Skilled in patient assessment and plan-of-care development
- Experienced with standardized assessments and outcome evaluation
- Proficient in wound care, manual techniques, therapeutic exercises, and assistive device training
- Able to administer modalities and implement evidence-based interventions
- Strong clinical documentation and regulatory compliance knowledge
- Excellent communication and interdisciplinary collaboration skills
- Familiarity with discharge planning, DME procurement, and supervising support staff
