Roles & Responsibilities
- Administer medications safely via oral, topical, inhalation, eye/ear and rectal routes under RN supervision
- Maintain and secure the medication cart and medication storage areas
- Perform counts of non-narcotic, countable medications at shift change with a med tech or licensed nurse
- Follow PRN medication protocols and obtain RN permission prior to PRN administration
- Document all medication administration accurately on the MAR according to facility policy
- Take and record vital signs, including apical pulse measurements, and report significant changes
- Coordinate with nursing staff to monitor medication supplies and reorder needs
- Provide compassionate CNA-level care and skilled nursing support to long-term care and rehab residents
- Serve in charge nurse responsibilities as assigned, communicating effectively with staff and supervisors
- Manage frequent interruptions while maintaining focus on resident safety and quality care
Qualifications
- Current LPN license (or Certified Nursing Assistant/Medical Technician as noted) required
- Experience in long-term care or skilled nursing environments preferred
- Familiarity with Medication Administration Record (MAR) documentation and med cart procedures
- Competence in taking vital signs and apical pulse measurements
- Ability to perform medication counts and follow PRN medication protocols
- Strong communication skills for working with residents, families, and interdisciplinary teams
- Ability to work independently, adapt to interruptions, and handle emotionally charged situations with professionalism
Education
- Licensed Practical Nurse (LPN) credential OR Certified Nursing Assistant/Medical Technician certification (CNA/CMT)
Knowledge & Skills
- Medication administration best practices and routes (oral, topical, inhalation, eye/ear, rectal)
- Medication cart management and secure storage procedures
- MAR documentation accuracy and compliance with facility policy
- Basic nursing assessments and vital sign measurement, including apical pulse
- Interdepartmental communication and reporting to charge nurse or supervisor
- CNA duties and resident-centered long-term care approaches
- Flexibility, personal integrity, and resilience in a fast-paced care setting
