Roles & Responsibilities

• Provide direct, patient-centered speech therapy services during home health visits
• Assess communication and swallowing function and develop individualized plans of care
• Implement evidence-based treatment plans using appropriate rehabilitation equipment and communication devices
• Collaborate with interdisciplinary team members to coordinate comprehensive patient care
• Document evaluations, progress notes and plan updates in a timely, professional manner
• Educate patients and caregivers on therapy goals, home programs and assistive devices
• Work independently with minimal supervision while maintaining high clinical standards

Qualifications

• Massachusetts Speech and Language Pathologist license (required)
• Certificate of Clinical Competence (CCC) or Clinical Fellow (CF) candidate accepted
• Minimum 2 years SLP home care experience (required)
• Master's or Doctorate in Speech-Language Pathology (required)
• Active American Heart Association BLS (AHA) certification (required)
• Valid driver's license and own reliable vehicle for home visits (required)
• One supervisory signed reference from within the past two years
• Current work history and Skills Checklist for Speech-Language Pathology (current to 1 year)

Education

• Master's or Doctorate in Speech-Language Pathology

Knowledge & Skills

• Expertise in assessment and treatment of communication and swallowing disorders
• Familiarity with rehabilitation equipment, language/communication aids and mobility devices
• Strong documentation skills and ability to develop patient-centered plans of care
• Comfort working independently in patients' homes with minimal supervision
• Ability to collaborate effectively with nurses, PT/OT and other interdisciplinary team members
• Strong time management and organizational skills for managing caseload and travel
