Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment for inpatient and outpatient populations, including orthopedics, neuro, wound care, lymphedema and pediatrics
• Develop individualized care plans and progress patients toward meaningful functional goals
• Collaborate closely with OT, SLP, OTA/PTA and rehab aides to deliver coordinated, patient-centered care
• Document thoroughly and accurately in Cerner and complete mandated forms (including OASIS for home health when applicable)
• Deliver safe acute care interventions on med-surg, telemetry and ICU floors and support transitions of care
• Support outpatient specialty services such as hand therapy and total joint rehab (knees, hips, shoulders)
• Participate in unit-specific orientation and ongoing quality improvement efforts

Qualifications

• BLS certification (required)
• Minimum: 2 years of clinical experience (as indicated)
• Experience in acute care and/or outpatient orthopedics preferred; home health experience preferred for home-based roles
• Comfortable treating total joints (knees, hips, shoulders), wound care and lymphedema
• Experience working with Cerner EMR; familiarity with OASIS for home health is a plus
• Ability to work collaboratively with multidisciplinary teams and adapt between inpatient and outpatient settings
• Willingness to work occasional weekends per contractual/quarterly needs

Knowledge & Skills

• Skilled in orthopedics, neuro, pediatrics and general acute care interventions
• Proficient with Cerner charting and documentation workflows
• Experience with hand therapy techniques in outpatient settings
• Knowledge of wound care and lymphedema management
• Familiarity with OASIS documentation for home health
• Strong communication, clinical decision-making and interdisciplinary collaboration skills
