Physical Therapy Assistant - SNF
Location: Watsonville, CA
Candidate Pay Rate: $68 - $47.6 per hour

Roles & Responsibilities
- Assist in implementing treatment plans developed by the supervising Physical Therapist to help residents regain independence
- Provide therapy focused on mobility, strength, balance, and functional tasks to improve quality of life
- Guide residents through therapeutic exercises and activities with compassion and clear instruction
- Monitor patient progress closely and report changes promptly to the supervising Physical Therapist
- Maintain accurate and timely therapy documentation to ensure continuity of care
- Collaborate with interdisciplinary care teams to coordinate comprehensive rehabilitation plans
- Educate residents and families on safe mobility techniques and fall prevention strategies
Qualifications
- Active California Physical Therapy Assistant (PTA) License required
- Associate Degree from an accredited PTA program required
- Previous SNF, rehabilitation, or geriatric therapy experience preferred
- Strong patient care mindset with excellent communication skills
- Ability to work effectively within a multidisciplinary healthcare team
Education
- Associate Degree from an accredited PTA program
Knowledge & Skills
- Experience supporting mobility training and rehabilitation programs
- Strong observation and patient monitoring abilities
- Effective documentation and record-keeping skills
- Clear, compassionate communication with residents, families, and care teams
- Comfortable working in a skilled nursing facility environment
