Location: Watsonville, CA

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and create individualized physical therapy treatment plans that promote functional independence
Provide hands-on therapy focused on mobility, strength, balance, and fall prevention
Monitor patient progress and modify plans to ensure measurable improvement
Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient-centered care
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Educate residents and caregivers on home exercise programs, safe mobility techniques, and strategies to prevent falls

Qualifications

Active California Physical Therapist License (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment, treatment planning, and patient care skills
Ability to work effectively within a multidisciplinary team and communicate clearly with residents and caregivers

Education

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Skilled in patient evaluation and individualized treatment planning
Proficient clinical documentation and electronic health record usage
Strong communication skills for educating patients, families, and team members
Knowledge of geriatric care principles and fall prevention strategies
