Roles & Responsibilities

• Provide skilled home health physical therapy visits and high-quality patient care
• Perform thorough assessments and develop individualized treatment plans
• Document patient visits and progress accurately using HCHB
• Coordinate care and communicate effectively with nurses, aides, and family members
• Deliver patient and caregiver education to improve safety and outcomes at home
• Manage a caseload (typically ~5–6 patient visits per day) with excellent time management
• Represent the agency professionally during home visits and community interactions

Qualifications

• 2+ years of experience (per job posting) — home health experience preferred
• Demonstrated home health therapy experience; hospice exposure a plus
• Proficient with HCHB documentation and electronic charting
• Reliable vehicle and willingness to travel to patient homes in Franklin / Macon County
• Available Monday–Friday for day shift coverage

Knowledge & Skills

• Strong clinical assessment and treatment planning skills in a home setting
• Comfortable with home safety and mobility evaluations
• Excellent documentation practices and familiarity with HCHB workflows
• Effective communication and patient/caregiver education skills
• Time management and route planning for multiple daily patient visits
• Collaborative approach for working with interdisciplinary home health teams
