Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living (ADLs)
• Measure, document, and report vital signs and patient status to the nursing team
• Assist with safe mobility, transfers, and positioning to promote patient comfort
• Accurately document care in the patient record and communicate changes timely
• Help maintain a clean, safe environment and adhere to infection control practices
• Collaborate with nursing staff and participate in team huddles to deliver coordinated care

Qualifications

• Current LNA / Licensed Nursing Assistant certification (VT/NH preferred or eligible)
• Completion of a state-approved nursing assistant training program or equivalent
• Reliable transportation (explicit requirement for this role)
• Current Basic Life Support (BLS/CPR) certification preferred
• Ability to pass background checks and required immunizations per facility policy
• Professional, patient-focused attitude with strong communication skills

Education

• High school diploma or GED and completion of a state-approved nursing assistant program
• Current LNA / Licensed Nursing Assistant certification

Knowledge & Skills

• Hands-on patient care and assistance with ADLs
• Taking and recording vital signs accurately
• Basic charting and documentation best practices
• Infection control procedures and safe patient handling
• Effective communication with patients, families, and clinical teams
• Time management and ability to work in a fast-paced care setting
