Roles & Responsibilities
- Deliver high-quality bedside nursing care for patients in the Uptown Health/Rehab area
- Collaborate with the interdisciplinary team to develop and implement care plans
- Monitor patient status, document changes, and escalate concerns promptly
- Administer medications and treatments safely, following facility policies
- Provide compassionate patient and family education to support recovery and discharge planning
- Support admissions, transfers, and discharge processes to ensure smooth patient flow
- Maintain accurate, timely documentation while adhering to regulatory standards
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing
- Basic Life Support (BLS) certification required; ACLS/PALS as appropriate or preferred
- Recent clinical experience in a nursing role preferred — rehab or acute care a plus
- Demonstrated ability to work night shifts and collaborate within a care team
- Up-to-date immunizations and ability to complete facility onboarding requirements
Education
- Vocational Nursing Diploma, Practical Nursing certificate or equivalent
Knowledge & Skills
- Strong clinical assessment and patient observation skills
- Comfortable with medication administration and documentation systems
- Clear communication with patients, families, and interdisciplinary staff
- Time management and prioritization during high-acuity or busy shifts
- Basic familiarity with rehab and post-acute care workflows is a plus
