Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients and support their daily needs
• Administer medications and treatments safely and document per facility protocols
• Monitor and record patient status, reporting any changes to the care team
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Educate patients and families about care plans and promote patient comfort
• Maintain a safe, clean environment and follow infection prevention standards

Qualifications

• Current LPN or T-LPN licensure (Licensed Practical Nurse)
• Active state nursing license or ability to obtain state licensure promptly
• Comfortable working in a team-focused long-term/post-acute care setting
• Willingness to complete facility onboarding, background checks, and required trainings

Education

• Completion of an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Medication administration and documentation proficiency
• Good communication and teamwork abilities
• Time management and prioritization in a busy care environment
• Familiarity with infection control practices and patient safety standards
• Basic electronic charting / EHR comfort
