Roles & Responsibilities

• Provide compassionate direct patient care as a Licensed Practical Nurse, making patients feel seen and heard
• Administer medications and treatments safely and accurately according to orders
• Perform routine assessments and communicate changes in patient status to the care team
• Document care thoroughly and timely to support continuity and quality of care
• Collaborate with RNs, providers, and support staff to deliver coordinated patient-centered care
• Support admissions, discharges, and patient education to improve outcomes and experience

Qualifications

• Current LPN license (eligible to practice in New Hampshire) — required
• Travel LPN (T-LPN) experience or willingness to work as a travel clinician is welcomed
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Clinical experience in a similar long-term care or skilled nursing setting preferred
• Valid professional references that speak to your clinical skills and teamwork
• Ability to pass facility onboarding checks (background, drug screen, immunizations)

Education

• Completion of an accredited Practical Nursing (LPN) program or diploma

Knowledge & Skills

• Strong medication administration and documentation skills
• Skilled in patient assessment, wound care basics, and ADL assistance
• Comfortable working within interdisciplinary teams and following care plans
• Good clinical judgment with the ability to escalate changes appropriately
• Clear communication skills with patients, families, and staff
• Adaptability to new environments and electronic charting systems
