Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents
• Administer medications and treatments safely and accurately
• Monitor and document vital signs, changes in condition, and care provided
• Collaborate with the interdisciplinary team to develop and follow care plans
• Educate patients and families about care, medications, and follow-up
• Promote a safe, clean environment and comply with infection control practices
• Respond promptly to patient needs and escalate concerns to providers

Qualifications

• Current LPN license in good standing (state licensure required)
• Basic Life Support (BLS/CPR) certification required; ACLS/other certifications a plus
• Comfortable working in long-term care/rehab settings
• Strong communication, clinical judgment, and teamwork skills
• Previous LPN experience preferred but new grads with clinical confidence are welcome

Education

• Completion of an accredited Licensed Practical Nurse program (diploma or certificate)

Knowledge & Skills

• Solid knowledge of medication administration and documentation
• Skilled in patient assessment and basic wound care
• Familiarity with electronic health records and charting best practices
• Ability to prioritize tasks, multitask, and remain calm under pressure
• Compassionate bedside manner and strong interpersonal skills
