Roles & Responsibilities

Provide compassionate, hands-on nursing care to walk-in clinic patients within the LPN scope of practice.
Take and accurately document vital signs and clinical observations to support provider decisions.
Triage patient phone calls, identify urgency, and coordinate appropriate follow-up or appointments.
Manage medication refill requests and complete necessary prior authorizations in a timely manner.
Act as the primary liaison between patients, clinical staff, and providers to ensure smooth care delivery.
Give direction to CNAs and Schedulers and collaborate closely with RNs, providers, and clinical managers.
Support the interdisciplinary team to meet patient needs and maintain high-quality clinic operations.
Maintain required licensure and complete continuing education to stay current with standards of care.

Qualifications

Graduate of an accredited Nursing Program (LPN diploma or equivalent) — required.
Current State LPN license (must hold a valid LPN license to practice).
BLS (Basic Life Support) certification from an accredited program — required.
Comfortable taking vital signs, triaging patient calls, processing medication refills and prior authorizations.
Able to give direction to CNAs and coordinate with schedulers and other clinic staff.

Education

Graduate of a Nursing Program (LPN diploma or equivalent)

Knowledge & Skills

Proficient in taking and interpreting vital signs
Skilled in telephone triage and patient communication
Experience managing medication refills and prior authorizations
Familiarity with clinic workflows and interdisciplinary collaboration
Basic life support (BLS) competency
Clear clinical documentation and care coordination abilities
