Roles & Responsibilities

• Provide compassionate nursing care to clinic patients within the scope of LPN practice
• Take and interpret vital signs and perform initial patient assessments
• Triage patient calls and prioritize care needs effectively
• Process medication refill requests promptly and accurately
• Prepare and complete prior authorizations to support timely patient treatment
• Serve as a primary liaison between patients, providers, and clinic staff to keep care coordinated
• Direct and supervise CNAs and coordinate with schedulers to ensure smooth clinic flow
• Collaborate with providers, coordinators, clinical managers/supervisors and RNs for optimal patient outcomes
• Document clinical interactions clearly and maintain accurate patient records

Qualifications

• Current State LPN license (required)
• Basic Life Support (BLS) certification (required)
• Graduate of a nursing program (practical/vocational nursing)
• Comfortable triaging patients, taking vital signs, and managing medication refills/prior authorizations
• Experience delegating to CNAs and working with interdisciplinary teams
• Follows continuing education and state/federal licensure requirements

• Graduate of a Nursing Program (Practical/Vocational Nursing)

Knowledge & Skills

• Patient assessment and vital signs measurement
• Telephone triage and clinical prioritization
• Medication refill processing and prior authorization workflows
• Clear clinical documentation and charting
• Strong communication with providers, coordinators and nursing leadership
• Delegation and supervision of CNAs and coordination with schedulers
