Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes, improving mobility and independence
Assess functional status and develop clear, goal-oriented treatment plans
Provide hands-on therapy, therapeutic exercise, gait training, and fall prevention strategies
Educate patients and caregivers on safe techniques and home exercise programs
Document clinical findings and progress thoroughly in the electronic record system
Collaborate with nurses, case managers, and the interdisciplinary team to optimize patient outcomes
Maintain compliance with home health regulations and best practice standards

Qualifications

Active state Physical Therapy license (required)
Home health experience preferred and valued
Current BLS/CPR certification or willingness to obtain
Valid driver's license and reliable transportation for home visits
Comfortable with clinical documentation and using VMS or similar systems

Education

Graduate of an accredited Physical Therapy program (DPT preferred; BS/MS where applicable)

Knowledge & Skills

Strong clinical assessment skills and the ability to create clear, measurable treatment plans
Skilled in therapeutic exercise, manual techniques, gait training, and fall prevention
Excellent communication and patient education skills for working with patients and families
Familiarity with home safety assessments and adaptive equipment recommendations
Proficiency using VMS systems and electronic medical records
