Roles & Responsibilities

• Carry out individualized treatment plans under the direct supervision of a Physical Therapist, delivering high-quality, hands-on care
• Assist in developing and updating plans of care that promote functional independence and patient safety
• Provide skilled restorative and maintenance therapeutic techniques and patient/caregiver education on each visit
• Assess patient status and progress at every visit and document outcomes clearly in the clinical record
• Coordinate care with interdisciplinary team members including nurses, physicians, caregivers, and rehab staff to ensure cohesive care
• Monitor and promptly report changes in patient condition to the case manager and supervising Physical Therapist
• Complete required clinical documentation and paperwork in compliance with federal and state regulations and APTA guidelines
• Stay current with HHCAHPS standards and integrate them into visit protocols to enhance patient experience

Qualifications

• Current Physical Therapist Assistant license or eligibility in the state of Vermont
• Comfortable working under the direct supervision of a Physical Therapist and following APTA guidelines
• Proven ability with restorative and maintenance therapeutic techniques and patient education
• Strong clinical documentation skills and familiarity with federal and state regulations
• Excellent communication and teamwork skills for collaborating with interdisciplinary care teams
• Home health or outpatient rehab experience is a plus

Education

• Associate degree from an accredited Physical Therapist Assistant program or equivalent completion of an accredited PTA curriculum
