Roles & Responsibilities
- Provide compassionate, hands-on rehabilitative care to residents in a skilled nursing facility
- Assist with implementing individualized treatment plans under the direction of the supervising PT
- Deliver safe transfers, gait training, and therapeutic exercise to improve function and independence
- Document resident progress clearly and timely to support continuity of care
- Collaborate closely with nursing and interdisciplinary team members to optimize outcomes
- Educate residents and families on home programs and safe mobility strategies
Qualifications
- Any experience level welcome — open to new grads and experienced PTAs
- Current or eligible Physical Therapist Assistant credential (check with recruiter for state specifics)
- Comfort working in a skilled nursing facility (SNF) setting
- Strong communication and teamwork skills to engage residents and the care team
- Reliable professional demeanor and commitment to resident-centered care
Knowledge & Skills
- Understanding of therapeutic exercise, transfers, and gait training
- Familiarity with SNF documentation practices and care plans
- Ability to follow PT-directed treatment plans and modify safely as needed
- Good clinical judgment, time management, and interpersonal skills
- Comfortable working in an interdisciplinary rehab environment
