Roles & Responsibilities
- Provide compassionate direct patient care to residents according to plan of care
- Administer medications and treatments safely and accurately
- Perform thorough patient assessments and monitor for changes in condition
- Document care, observations and communications clearly in the medical record
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Educate residents and families about care plans and answer questions with empathy
- Support a safe, clean and respectful environment for residents and coworkers
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Current basic life support CPR certification preferred
- Comfortable working in a residential/long term care setting and supporting team-based care
- Strong communication skills and a patient-first attitude
Education
- Graduate of an accredited practical nursing program for LPN or an accredited nursing program for RN
Knowledge & Skills
- Skilled in patient assessment and basic nursing procedures
- Accurate medication administration and documentation
- Familiarity with infection control practices and safe patient handling
- Good time management, prioritization and teamwork skills
- Comfortable using electronic health records for charting
