Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and follow care plans accurately and safely
- Perform routine assessments and monitor patient status
- Document care thoroughly in the electronic health record
- Communicate clearly with the care team and families to coordinate care
- Assist with activities of daily living while promoting resident dignity and independence
- Participate in care planning and quality improvement initiatives
Qualifications
- Current Licensed Practical Nurse (LPN) license (Pennsylvania) required
- Graduate of an accredited LPN program
- Basic Life Support (BLS/CPR) certification preferred or willing to obtain
- Strong communication and teamwork skills
- Comfortable working evening and night hours
Education
- Graduate of an accredited Licensed Practical Nurse (LPN) program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic patient assessment and monitoring
- Familiarity with infection control and safety protocols
- Experience with electronic health records and clinical documentation
- Ability to provide compassionate patient and family education
