Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and according to policy
• Perform thorough assessments and monitor changes in condition, escalating when needed
• Document patient status, interventions and outcomes in a clear and timely manner
• Collaborate with nurses, therapists and facility staff to deliver coordinated care
• Educate residents and families about care plans and best-practice self care
• Support admissions, discharges and transitions to ensure continuity of care
• Maintain infection control, safety standards and a clean therapeutic environment

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Hampshire or ability to obtain prior to start
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) certification required; advanced certifications a plus
• Comfortable with medication administration, charting and resident assessments
• Previous experience in long term care, skilled nursing or similar setting preferred
• Strong communication skills and a team-oriented, resident-centered approach

Education

• Diploma or certificate from an accredited Practical Nursing program
• High school diploma or equivalent required

Knowledge & Skills

• Skilled in medication administration and documentation
• Proficient with resident assessments and vital signs monitoring
• Able to prioritize tasks in a fast-paced environment
• Familiar with infection prevention and safety protocols
• Good interpersonal skills for working with residents, families and clinical teams
• Basic computer/charting competency for electronic health records
