Shift Type: Friday, 3:00 PM - 11:00 PM; Saturday-Sunday, 7:00 AM - 11:00 PM (rotating day and evening weekend schedule)

Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, focusing on safety and dignity
- Administer medications and treatments accurately and according to policy
- Perform thorough assessments and monitor changes in condition, escalating when needed
- Document patient status, interventions and outcomes in a clear and timely manner
- Collaborate with nurses, therapists and facility staff to deliver coordinated care
- Educate residents and families about care plans and best-practice self care
- Support admissions, discharges and transitions to ensure continuity of care
- Maintain infection control, safety standards and a clean therapeutic environment
Qualifications
- Current Licensed Practical Nurse (LPN) license in New Hampshire or ability to obtain prior to start
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification required; advanced certifications a plus
- Comfortable with medication administration, charting and resident assessments
- Previous experience in long term care, skilled nursing or similar setting preferred
- Strong communication skills and a team-oriented, resident-centered approach
Education
- Diploma or certificate from an accredited Practical Nursing program
- High school diploma or equivalent required
Knowledge & Skills
- Skilled in medication administration and documentation
- Proficient with resident assessments and vital signs monitoring
- Able to prioritize tasks in a fast-paced environment
- Familiar with infection prevention and safety protocols
- Good interpersonal skills for working with residents, families and clinical teams
- Basic computer/charting competency for electronic health records
