Roles & Responsibilities

• Deliver compassionate direct patient care, ensuring dignity and respect for each person
• Administer medications and treatments accurately and safely
• Monitor and document patient vital signs and clinical changes in a timely manner
• Collaborate with the interdisciplinary team to coordinate excellent care
• Perform wound care, basic procedures and nursing interventions according to scope of practice
• Educate patients and families on care plans and discharge instructions
• Maintain accurate, organized charting and follow facility policies and protocols

Qualifications

• Active LPN or RN licensure (candidate must be eligible to practice in New Hampshire)
• Current BLS (Basic Life Support) certification or ability to obtain prior to start
• Previous experience in long-term care or med-surg settings preferred
• Ability to work Monday-Friday daytime schedule and adapt to facility policies
• Strong communication skills and a team-first attitude

Education

• LPN: Graduate of an accredited practical nursing program
• RN: Graduate of an accredited associate or bachelor nursing program

Knowledge & Skills

• Solid clinical assessment and patient monitoring skills
• Medication administration and documentation proficiency
• Comfortable with electronic health record charting
• Time management and prioritization in a busy clinical setting
• Patient- and family-centered communication and education
