Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled rehab and nursing environment
- Administer medications and treatments accurately and document care in a timely manner
- Monitor resident status, report changes to the interdisciplinary team, and participate in care planning
- Assist with activities of daily living while promoting independence and dignity
- Collaborate with nurses, therapists, and support staff to deliver person-centered care
- Educate residents and families on care plans, medications, and wellness strategies
Qualifications
- Valid LPN license for Massachusetts (required)
- Completion of an accredited practical nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong communication, teamwork, and documentation skills
- Professional, patient-focused attitude with sound clinical judgment
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Knowledge of basic nursing procedures, medication administration, and wound care
- Comfortable with electronic charting and accurate documentation
- Ability to assess resident condition and escalate concerns appropriately
- Skilled in time management and prioritizing resident needs during a busy shift
- Familiarity with infection control, safety protocols, and resident rights
