Roles & Responsibilities

• Provide compassionate, resident-centered care at the bedside
• Administer medications and treatments safely within LPN scope
• Monitor and document vital signs, changes in condition, and care provided
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Assist residents with activities of daily living while promoting independence
• Support admissions, discharges, and care transitions with accurate documentation

Qualifications

• Current and active LPN license in Kentucky (or compact state privileges as applicable)
• Completion of an accredited Practical Nursing program
• BLS/CPR certification required; additional certifications a plus
• Reliable, compassionate approach with strong clinical judgment
• Ability to pass background check and drug screening per facility policy
• Prior long-term care or rehab experience preferred but new grads welcome with strong clinical skills

Education

• Graduate of an accredited LPN/LVN program; High school diploma or GED required

Knowledge & Skills

• Solid medication administration and documentation practices
• Skilled in basic patient assessment and infection control
• Comfortable using electronic medical records and charting accurately
• Strong communication and teamwork skills with residents and families
• Ability to prioritize tasks in a fast-paced long-term care environment
• Basic wound care knowledge and safe transfer/positioning techniques
