Roles & Responsibilities
- Evaluate and assess patients' functional needs and develop individualized occupational therapy plans
- Provide hands-on treatment and adaptive strategies to help residents regain independence
- Collaborate with an interdisciplinary therapy team to coordinate patient-centered care
- Document clinical progress and maintain required productivity and quality standards (90% productivity expectation)
- Follow Continuum Rehabilitation Group policies and procedures to ensure consistent, compliant care
- Participate in team meetings and contribute to a positive, resident-focused therapy environment
Qualifications
- OT — occupational therapy skillset (position listed as OT)
- 1-3 years clinical experience preferred (years of experience listed as 1-3)
- Experience in a skilled nursing facility (SNF) or similar post-acute setting highly valued
- Comfort with onsite, in-person practice; therapist must be present in the building to receive guaranteed hours
- Willingness to follow facility dress code (black scrubs) and employer policies
- Able to participate in an interview with the Regional Director of Therapy (contact provided in job notes)
Knowledge & Skills
- Skilled in occupational therapy evaluations and treatment planning for SNF residents
- Strong documentation practices and familiarity with productivity expectations
- Ability to work collaboratively within a multidisciplinary therapy team
- Good clinical judgment and resident-centered problem solving
- Professional communication with residents, families, and facility staff
- Adaptability to facility policies and established workflows
