Shift Type: Rotating day/evening shifts with rotating weekends. Monday-Friday: start at 6:30 AM or 8:00 AM (day/evening rotation); Rotating weekends: Saturday/Sunday 7:00 AM - 12:00 PM; 40 hours per week, rotating

Roles & Responsibilities
- Interpret medication orders and maintain accurate computerized patient medication profiles
- Prepare and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
- Manage controlled substances and maintain compliant records per federal and state regulations
- Monitor drug therapy for interactions, contraindications, allergies, and dosing appropriateness
- Perform pharmacokinetic dosing per hospital protocol to optimize patient outcomes
- Review and interpret culture and sensitivity data to support antibiotic stewardship
- Document clinical activities and interventions clearly and timely
- Provide clinical consultation and drug information to practitioners, patients, and families
- Participate in quality improvement, medication reviews, and departmental education efforts
- Serve as an active member of the Code Response Team when needed
Qualifications
- Active Colorado Pharmacist License (CO PHA) in good standing
- Graduate of an ACPE-accredited School of Pharmacy, BS Pharmacy required
- Epic experience is required; Meditech training is a strong plus
- BLS (American Heart Association) required
- ACLS (American Heart Association) required
- Influenza vaccination required during the designated season (Oct 1 - Apr 1), no exemptions
- Experience with sterile compounding, chemotherapy preparation, and parenteral nutrition
- Proven ability in medication dispensing, pharmacokinetic dosing, and controlled substance management
- Strong documentation skills and experience participating in quality improvement activities
Education
- BS Pharmacy from an ACPE-accredited school
Knowledge & Skills
- Solid clinical hospital pharmacy and medication safety knowledge
- Hands-on experience with Epic and familiarity with EMR transitions
- Competence in sterile compounding and IV admixtures
- Pharmacokinetic dosing and drug therapy monitoring skills
- Antibiotic stewardship and culture & sensitivity interpretation
- Formulary management and therapeutic substitution practices
- Clear communication for interdisciplinary collaboration and patient education
- Comfort working in fast-paced inpatient settings and participating in code response
