Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Deliver exceptional physical therapy services to patients in their home settings
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with patients, families, and healthcare teams to achieve optimal outcomes
Document patient progress and adjust treatments as needed
Educate patients and caregivers on exercises and safety techniques

Qualifications

Active California Physical Therapy license required
Current Basic Life Support (BLS) certification (AHA or equivalent)
Previous home health experience preferred
Ability to travel within assigned area (25% travel required)

Knowledge & Skills

Expertise in physical therapy evaluation and treatment
Knowledge of home health regulations and best practices
Strong communication and interpersonal skills
Ability to work independently and manage a caseload
