Shift Type: Day shifts, Monday-Friday, no weekends or holidays

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start-of-care visits and develop individualized treatment plans
Collaborate with interdisciplinary team to ensure optimal patient outcomes
Document patient progress using HCHB EMR system
Educate patients and families on therapeutic exercises and home safety

Qualifications

Active Physical Therapy license in the state of California
BLS certification from the American Heart Association
Minimum 1 year of home health experience (new travelers with less experience welcome to apply)
Proficiency with HCHB (HomeCare HomeBase) EMR

Knowledge & Skills

Expertise in orthopedic and geriatric rehabilitation
Skilled in managing patients with multiple comorbidities such as CHF and COPD
Ability to perform comprehensive assessments and create effective treatment plans
Excellent communication and documentation skills
