Roles & Responsibilities

• Assess, evaluate, and update therapeutic/rehabilitative/functional status, and participate in developing the total plan of care.
• Evaluate home environment and assess muscle strength, mobility, gait, ROM, and potential for rehab.
• Direct physical therapy treatment and instruct patients and families in the use and care of therapeutic appliances.
• Report patient reactions to treatment or changes in condition to the physician.
• Initiate physical therapy programs and instruct other personnel and family members in certain phases of therapy.
• Supervise homemaker/Home Health Aides as they participate in the patient's rehabilitation regime.
• Identify patient and family needs for other home health services and refer as necessary.
• Prepare and submit clinical and progress summaries, observing and recording patient condition and reporting to the attending physician.
• Participate in peer consultation and ongoing staff competency assessment.
• Comply with Title 22, Medicare Conditions of Participation, Medi-Cal, OSHA, and other regulations.

Qualifications

• Minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting.
• Current BLS certification required.
• Valid Physical Therapy license in California.
• Good oral and written communication skills.
• Strong organizational skills.
• Ability to work independently and as part of a team.

Knowledge & Skills

• Expertise in physical therapy assessment and treatment planning.
• Knowledge of home health regulations and Medicare Conditions of Participation.
• Proficiency in patient education and family instruction.
• Excellent documentation and record-keeping skills.
• Ability to supervise and collaborate with other healthcare personnel.
• Familiarity with Title 22 and OSHA requirements.
