Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); plus minimum 2 weekend days per month (typically one Saturday and one Sunday in separate weekends) — Day shift with occasional floating to other Swedish campuses as needed

Candidate Pay Rate: $52 per hour

Roles & Responsibilities

• Perform thorough biopsychosocial assessments to develop person-centered plans of care
• Coordinate discharge planning and arrange home care, equipment, and referrals
• Document clearly and promptly in Epic to support coordinated care and transitions
• Provide crisis intervention and high-risk patient safety planning when needed
• Collaborate with interdisciplinary teams including PT, respiratory, pharmacy and nursing
• Facilitate care conferences and referrals to rehab, long-term care, and community resources
• Assess needs for DME, Medicaid/Medicare supports, and prior authorization processes
• Engage patients and caregivers around advance directives and guardianship/power of attorney issues

Qualifications

• BLS required
• LSWAIC required
• Epic experience required (charting system proficiency)
• Minimum 1 year inpatient/acute care case management experience
• LCSW or MSW credentials preferred
• Comfort floating to nearby campuses as census dictates
• Experience with crisis intervention, discharge planning, and community resource navigation

Education

• MSW (Master of Social Work) preferred
• LCSW credential preferred or eligible
• BLS certification required

Knowledge & Skills

• Biopsychosocial assessment and developing plans of care
• Care coordination and collaborative discharge planning
• Advance directives, guardianship, and protective services knowledge
• Familiarity with Medicaid, Medicare, prior authorization, and community programs
• Crisis intervention and suicide/homicide risk assessment
• Documentation best practices and Epic charting
• Cultural competence and working with diverse patient populations
• DME needs assessment and referral processes
